Internal Salesforce users from within Salesforce

Hi All,I am wondering if anyone has ever surveyed their Salesforce user base from within Salesforce. I have been tasked to find a way to survey a specific team (1 profile) but the catch is I can not email or text them.The survey has to be displayed and completed from within Salesforce.I looked at chatter polling but the business does not like the fact that it is anonymous and that the poll results are shared with the user. The business feels this could impact the results. Also, they want to pull reports on who completed the surveys and when the survey was completed.I am thinking this will require a custom object with a relationship to the user object. Just looking for ideas and if anyone has done anything similar.
